Transition

This program will provide support to school district staff who are responsible for designing and implementing transition plans for students with disabilities aged 14-21. 

The CESA #12 transition consultant will provide support to districts including, but not limited to: transition assessment, transition program development, networking, identifying community and county resources to support secondary transition, and student-specific services where applicable.
